You don’t need a prior relationship with Ophthalmology Limited in order to refer a patient. Once our appointment schedulers receive the referral, they work with our team of ophthalmologists to connect your patient with the right specialist for their needs.

  • Download, complete, and submit the form
  • To best treat patients, we request that previous records, tests, treatments and a copy of the patient’s insurance card, be submitted with the referral request
